About Rural Health Connections:
Rural Health Connections is a community-focused initiative by Rice Community Health, aiming to empower the citizens of Rice County and central Kansas. Our dedicated team works to:
- Improve Population Health: Connecting high-risk individuals with essential community resources and clinical services.
- Provide Education: Offering diabetes education and hypertension management to those most in need.
- Community Outreach: Engaging in education about COVID-19, chronic disease screening, and overall health, especially in areas with high rates of these conditions.
- Support New Parents: Providing postpartum assistance to ensure families thrive.
By participating in this event, you're directly supporting these vital programs, ensuring that our neighbors have access to the care and resources they need to lead healthy lives.
